If you would like to access your medical records, we invite you to please log onto your patient portal. From there you can view and print recent visit notes and test results.
If you would like a copy of your medical record to be sent to you or another provider, please complete the following MEDICAL RELEASE FORM and fax it to (847) 459-7929.
Please be advised, there is a processing fee if records are to be sent to yourself. There is no fee if records are to be sent to another provider's office. All fees can be found at the bottom of the release form.
